IBM's Institute for Business Value surveyed 2,000 technology C-suite executives globally, producing a picture of AI governance lagging badly behind deployment speed. Two-thirds of respondents are held accountable for AI systems where they lack full visibility. Seventy percent say business teams are deploying AI faster than IT can track. CEO-driven AI transformation mandates are in place at 80% of organizations — creating an accountability gap between what leaders are responsible for and what they can actually control. The agent-specific incident data is notable: organizations experienced an average of 54 AI agent incidents last year, with 17% classified as high-severity (requiring more than 4 hours to contain). Of those high-severity incidents: 37% resulted in data exposure or security breaches, 33% caused cascading system failures, and 17% triggered compliance violations. Only 11% of surveyed technology leaders believe they are fully prepared for the AI agent deployment scale they expect within the next year. The single strongest governance finding: organizations that embed control directly into their AI system architecture — rather than applying manual oversight afterward — experience 25% fewer incidents. AI IT budgets are projected to rise from 15% of total IT spend in 2025 to nearly 25% by 2027. This is an IBM-commissioned study, introducing some selection bias risk — IBM sells governance tooling.
